I/we agree to allow my/our child to participate in all camp activities. I/we hereby apply for registration for the herein named child for the camping services indicated in this application. In consideration of acceptance of this application by Hoops Skool, I/we hereby agree as follows:

(a) that the directors of Hoops Skool reserve the right to terminate the registration of any athletes when it is deemed by the directors to be in the best interests of the child or the camp. In such an event it is understood that a proportionate refund will be made.

(b) to make all payments by cheque in accordance with the fee schedule in effect on the date of registration. (2% service per month service charge on unpaid balance after May 15, 2006. NSF cheques are subject to $25 service charge and certified funds will be required.)

(c) to give camp officials authority to act on my behalf in case of emergency.

(d) to release and indemnify Hoops Skool from any and all claims for losses of articles and damages arising as a result of any accident, injury or otherwise sustained by the herein named child arising from participation in any camp activities and attendance at and travel to/from camp

(e) to consent to the use by Hoops Skool of the athlete's likeness for publicity purpose relating to camp.
